I understand that in order to be eligible for driving lessons in Phase 2, I must have a valid learner's permit (permis d'apprenti or permis provisoire) and have completed Module 6. I agree to complete Phase 2 within 2 months from its start.

I understand that in order to be eligible for Module 10 in Phase 3, I must have completed Module 8 and Module 9. Driving lessons 5, 6, 7 and 8 must be completed before taking Module 10. I agree to complete Phase 3 within 4 months from its start.
 

I understand that in order to be eligible for Module 11 in Phase 4, I must have completed driving lessons 9 and 10 of Phase 3. I agree to complete Phase 4 within 4 months from its start.

The next agreed payment installment will have been paid at the end of each phase, or will paid with 3 days of receipt of this attendance form.

I am aware that there is a maximum of 21 persons allowed per Module session and that I am eligible based on a registration date priority basis. Earliest registrations are eligible first. First come, first served. I understand that later registrations and late confirmations will be placed on a waiting list. A registration will only be confirmed if the agreed payment installment has been made.

I am aware, and I am in agreement, that an additional fee will be charged to make up the missed Module or Modules if I do not notify City Driving School to cancel with a minimum of 48 hours (weekends not included) notice. Failure to attend on confirmed dates may not guarantee attendance for future dates. An additional fee may be required to make up the missed class. Email or voicemail cancellations are not accepted."

City Driving School does not accept responsibility for any disqualifications due to failure to attend applicable modules in their proper order. The student assumes all responsibility to be informed of and to have the necessary prerequisites for all applicable driving lessons within each Phase. Any driving lessons taken in improper order with respect to the Road Safety Education program will be considered as supplemental lessons and will be charged as such.
